Congratulations to Thanh Pham, MBA, BS,
RPh, who was promoted to the Pharmacy Team Leader-Clinical Operations
position at PAH. Thanh first joined Pennsylvania Hospital in 2004 as a clinical
pharmacist. His prior and diverse experience includes working as a clinical
pharmacist at a large commercial health plan, a national retail drug chain, as
well as a local university-based health system.
“During his time with us, Thanh has been an
integral team member participating in many initiatives including the BOOST
program to reduce congestive heart failure readmissions and pharmacist-managed
Core Measure compliance,” said Suzanne Y. Brown,
MS, RPh, director of Pharmacy Services.
Thanh has demonstrated a record of leadership both interdepartmentally
as well as across disciplines, as he was instrumental in the development of the
hospital’s Alaris pump library and most recently was selected as one of the
Educational Leads for the Knowledge Based Charting enterprise-wide implementation.
“Thanh’s firm understanding of inpatient and outpatient operations, coupled
with his depth of knowledge in clinical, technical and operational initiatives,
enhance the Pharmacy leadership team as the department continues to offer
expanded services to our patients and the hospital,” added Brown.
Thanh’s responsibilities will include
coordinating clinical and operational support for the department; ensuring
daily operational services meet expectations; facilitating communication and
education within the department; and participating in performance improvement
initiatives related to medication management.
Thanh
received his Bachelor of Science degree in Pharmacy from Temple University and
his MBA from Rosemont College.
